Copy of Driver’s License or California ID
Copy of Social Security Card
Copy of your THREE most recent pay stubs

There is a screening fee of $45 per applicant. The fee is to be paid in cash, money order, or credit card over the phone (no personal checks). There is a $2 credit card transaction fee.
